Pediatric (model for) End-stage Liver Disease (PELD) is a disease severity scoring system for children under 12 years of age used by the United Network for Organ Sharing (UNOS) for prioritizing allocation of liver transplants in juveniles.: scores for patients listed for liver transplantation before the patient's first birthday continue to include the value assigned for age <1 until the patient reaches the age of 24 months It is calculated from the patient's:
albumin
bilirubin
international normalized ratio (INR)
age at listing
degree of growth failure (based on gender, height and weight)
PELD = 4.80[Ln serum bilirubin (mg/dL)] + 18.57[Ln INR] - 6.87[Ln albumin (g/dL)] + 4.36 (if patient is < 1 year old) + 6.67 (if patient has growth failure of <-2 Standard Deviation)
You can do the math yourself or link to an excellentMELD/PELD calculator provided by UNOS.
If the patient is less than one year old at the time of listing, he/she will continue to include the value assigned for age <1 until the patient reaches the age of 24 months
The same modifications, mortality predictions and exceptions as used on MELD scores are used for PELD scores as well. The end value of the score is what is used in determining priority when an organ becomes available._
